Group Dental Claims
We know providers, employers and employees expect quick claims resolution, and we deliver. We accept dental claim forms from providers, or employees can obtain claim forms from an employer. Employees can access summary of benefits and claims from their smartphone or mobile device with our Principal MobileSM application or on the Member Web site by clicking the Claim Activity tab under Health, Dental & Vision.
Additional Benefits
At-risk members can receive extra care, and all members can get a second opinion. Qualifying members take the appropriate voucher to their dental providers. For correct claim processing, the provider submits the voucher and claim form to Principal Life.
- Periodontal Program – Provides additional benefits for members who are pregnant, or who have diabetes or heart disease.* Get voucher.
- Cancer Treatment Oral Health Program – Provides extra care for members undergoing chemotherapy and/or head/neck radiation therapy. Get voucher.
- Second Opinion Program – Ensures members receive the best advice about dental care by covering second opinions. Get voucher.
