Group Critical Illness Claims
Because critical illness claims take place at times that are difficult for claimants, we make it a point to make the claims process simple, give these claims priority and handle claims as quickly as possible.
Critical illness claims can be filed in a variety of ways including email, fax or mail. Members may download a claim form from the Claim Activity tab under Insurance on the Member website. To request payment under the Wellness benefit, members complete a web submission form on the Member website. Critical illness Explanation of Benefits can be viewed on the Member web site by clicking the Claim Activity tab under Insurance.
